The definitive tracker for America's $50B rural health rebuild

The Rural Health Transformation Program is a $50 billion commitment from CMS, spread across all 50 states over five years, to rebuild the infrastructure of rural American healthcare. Every state is writing its own playbook right now — issuing RFPs, awarding contracts, standing up programs, picking vendors. Most of that activity is happening in public, but it’s scattered across hundreds of state agency pages, procurement portals, board agendas, PDFs, and press releases.

What we track. Every state’s RHTP application, award notice, solicitation, legislative action, and implementation milestone. We watch the official sources — governors’ offices, state health departments, Medicaid agencies, procurement systems — and we watch the news. When something changes, we capture it. When a state publishes a project narrative, we read it and extract what matters: who’s doing what, with whom, for how much, and by when.

How we work. The tracker is built on a pipeline that combines automated monitoring of hundreds of official and news sources with hands-on editorial review. Everything we publish is sourced and dated. When we don’t know something, we say so. When a state changes direction, we note it and link to it.

Where this is going. The newsletter is the front door. Behind it, we’re building a structured intelligence layer — a canonical record of every prime vendor, contract, opportunity, and contact associated with RHTP implementation. That work will surface in progressively richer ways over the coming months: deeper state profiles, a searchable dashboard, and eventually a data feed for teams that need to operate on this information at scale.

O’Neil through Civic Operator, a Chicago-based consultancy building practical strategies for integrating AI, automation, and emerging technologies to drive impact. The project reflects a long-running conviction that public programs work better when the public — and the people inside them — can actually see what’s happening.
