Application deadline is April 24, 2026.
The Department of Vermont Health Access (DVHA; the State) is seeking grant applications for Mobile Health Care Clinics with a focus on activities related to one or more of the following health care domains: health education, preventative primary care, dental care, maternal and child health care, mental health care, substance use disorder treatment, chronic disease management, and emergency stabilization services to expand access to care to some of the hardest to reach Vermonters. Preference will be given for proposals that are able to demonstrate, using available data, a clear gap in care specific to a region of the state or target population.

Legislation Watch
Arizona SB1316: Establishes the Arizona Rural Health Transformation Fund as the state vehicle for receiving and distributing Arizona’s $167 million annual RHTP allocation from CMS. The bill passed the Senate 17-13 on February 10, crossed to the House, and cleared the House Health & Human Services Committee 6-5 on March 23 and the House Rules Committee 8-0 on March 30. It awaits a full House floor vote. (Arizona Legislature)
Iowa HF2743: Creates the Iowa Rural Health Transformation Fund and appropriates authority to the Iowa Department of Health and Human Services to distribute the state’s $209 million first-year RHTP award — the largest per-capita allocation in the program. Iowa was the first state to begin awarding RHTP subgrants under its “Healthy Hometowns” initiative, and this bill codifies the fund structure and spending authority that Governor Reynolds’ administration has been operating under by executive action. It was read for the first time and referred to Appropriations on March 20. (KCRG)
