Missouri announces 27 Hub Anchor awardees and Rural Health Transformation Funding for Home Visiting

Missouri’s Rural Health Transformation Office received 41 Hub Anchor applications, representing all 27 ToRCH Care Community Hub Areas, from a variety of organizations, including hospitals, FQHCs, behavioral health organizations, local public health agencies, nonprofits, and universities. The strong response reflects broad engagement from organizations across Missouri committed to improving the health of their rural communities through collaboration and local leadership. Applications were evaluated through a competitive review process using standardized criteria that assessed organizational readiness, governance capacity, community partnerships, staffing plans, operational capabilities, and alignment with the goals of Missouri’s RHTP.

Following this competitive review process, one Hub Anchor organization was selected to serve each ToRCH Care Hub. Formal participation in the Rural Health Transformation Program as a Hub Anchor is subject to execution of a Hub Anchor Participation Agreement. The selected 27 organizations are expected to help lead implementation of the ToRCH Care model by convening local leadership, fostering collaboration across healthcare providers and community partners, and supporting sustainable approaches to improving health outcomes in rural Missouri.

Separately, a new funding opportunity opened: Rural Health Transformation Funding for Home Visiting, administered through the Missouri Children’s Trust Fund.

Rural Health Transformation Funding for Home Visiting – Application Period Through August 23, 2026

The Missouri Children’s Trust Fund (CTF) is pleased to announce the total availability of $588,000 annually to support agencies in initiating and implementing Rural Health Transformation (RHT) home visiting. This funding comes from the Rural Health Transformation Program, administered by the Department of Social Services, MO Healthnet Division. The inclusion of home visiting in the Rural Health Transformation Program’s spending plan reflects our state’s ongoing commitment to home visiting as a strategy to improve maternal and child health.

Grantees will have the option to renew their contracts annually for up to 4 additional years, subject to performance and funding availability. Funding will allow up to $100,000 for model fees and training costs for new nurse-based home visitors and up to $488,000 for the expansion of nurse-based home visiting.

Eligible home visiting programs must currently be implementing, or working toward implementing, an evidence-based, HomVEE-approved model and be able to demonstrate fidelity to the model. All services must be provided by nurses. Services must be an expansion and cannot supplant current funding. Additional eligibility requirements are outlined in the RFP and attachments below:

Oklahoma extends a deadline and a series of invitations

Also: the EMS & Community Paramedicine Vehicles NOFO deadline was extended from July 20 to July 27, 2026 (11:59 p.m. CT), via a posted amendment (Jul 17).

On engagement, OSDH published a large slate of “RRR Regional Kickoff” events across its health districts, running July 27–28 and beyond — regional meetings with named facilitators and specific venues (Kellyville/Creek County Fairgrounds, Woodward/High Plains Technology Center, Elk City/Beckham County Health Department, and more). Worth a short “Oklahoma takes the program on the road” note, and these are calendar-worthy if you flag events.

Vermont clarifies

Vermont clarified its rolling application windows: applications received by 7/17 (10 p.m.) are considered for a September 1 start; applications received by 8/17 (10 p.m.) are considered for an anticipated October 1 start — so the next live cutoff is August 17.
